A completed family waiver must be submitted prior to participation in any clubhouse activity.<\/strong><\/h4>\n\n\n\n

  2. Enter child\/ren\u2019s information on page two;<\/li>\n\n\n\n
  3. Select \u201cNew minor athlete\u201d button on the page;<\/li>\n\n\n\n
  4. Enter your (parent) information on page one;<\/li>\n\n\n\n
  5. Read waiver carefully and check each box;<\/li>\n\n\n\n
  6. Enter Emergency Contact Information;<\/li>\n\n\n\n
  7. Sign in the box provided;<\/li>\n\n\n\n
  8. and click \u201cAccept\u201d<\/li>\n<\/ol>\n\n\n\n
